Before your first visit

• Bring photo ID and your insurance card.
• Bring any imaging reports or referral paperwork you have.
• For a work injury or car accident, bring your claim information.
• Wear something you can move in, with access to the area being treated.
• Arrive a few minutes early if it is your first time — there is paperwork.

Forms

Completing your intake before you arrive means more of your first appointment is spent on the evaluation rather than on a clipboard.

Build note: downloadable forms are not wired up yet. Intake that collects symptoms or insurance member IDs is PHI and belongs in the compliant intake app (Phase 5), not as a PDF on the static host. Blank, non-personalised PDFs can live here safely; completed ones cannot.
