The first 48 hours after an ankle sprain

Medically reviewed by Ralph Cuartero, PT, DPT

Ankle sprains are the most common musculoskeletal injury there is, and among the most commonly under-treated. Most people rest until it stops hurting, return to normal, and then sprain the same ankle again within a year.

Move it, carefully, early

Complete rest is no longer the advice. Gentle movement within a comfortable range, started early, helps swelling drain and keeps the joint from stiffening. Total immobilisation costs you range of motion that then takes weeks to get back.

Load it as soon as it tolerates load

Walking on it, as normally as you can manage, is usually better than avoiding weight entirely. If you are limping heavily, that is a sign to reduce the distance rather than to stop moving.

The part almost everyone skips

After a sprain, the ankle stops reporting its position accurately. That is why re-injury rates are so high -- the joint is strong enough, but it does not know where it is. Balance and proprioception training fixes this, and it is almost always the piece that gets left out.

If you have sprained the same ankle more than once, that missing piece is probably why.
